Notes:	Location: Superior Ambulance Campus 25400 W. Eight Mile Road – between Telegraph road and Beech Daly Southfield, Mi 248-304-6057 IMPORTANT: This course must be scheduled on a date/time that is acceptable both to you and Life Support Training Institute. You must call 248-304-6057 to arrange a course date/time. IMPORTANT: The class will be held in the LSTI building, 3rd building on the east side of the Superior Ambulance Southfield campus (old Community EMS), next to the Citgo gas station. Course fee includes: ACLS Provider manual, ACLS Instructor manual, ACLS course DVD, classroom instruction, evaluated teach-back session, ACLS instructor card COURSE PREREQUISITE: Prior to the above course date, you must complete the AHA ACLS Instructor Essentials course at https://shopcpr.heart.org/acls-instructor-essentials-online You must bring the Instructor Essentials completion certificate, along with a copy of your current ACLS card to class.
